Online Application for Clearance

PROCEDURE FOR ONLINE APPLICATION FOR CLEARANCE (EXCEPT COLLEGE OF MEDICINE)

(NOTE: Students of colleges or departments that already have an established procedure for accomplishing the clearance form should follow their college’s/department’s procedure.)

Process Flow:

Step 1

Download the applicable Clearance Form by clicking the appropriate filename below.

• If you already obtained the hard copy of the appropriate clearance form from the University Registrar’s Office, skip this step and proceed to step 2.
• If you wish to fill out the downloaded form directly on the device, you may do so, then proceed to Step 4. Otherwise, print the downloaded form then proceed to step 2.

Step 2

Fill out the form legibly with the relevant data.

Step 3

Scan the filled-out form by using a flatbed scanner or phone app such as CamScanner, Genius Scan, or similar. Important: Forms that are of less than optimal image quality will not be processed.

Save the file in PDF format using the filename Lastname Firstname M.I.clr.pdf (Example: de la Cruz Juan B.clr.pdf).

Email the PDF file to your Dean’s/Principal’s Office from an email address that you can monitor on a daily basis. (Click here for a list of relevant office email addresses.)• Use the Email Subject Line: Application for Clearance – Lastname, Firstname M.I.• To avoid the rush, email the PDF file within 24 hours from the time you download the form

Step 4

Send your school ID by courier to:

• You will not be able to complete your clearance unless you surrender your school ID to the University.
• If you lost your ID, send an original copy of the affidavit of loss by courier to the Head of SAPPRO.

Step 5

Wait for an email from the University that will inform you that either a) you have been cleared of all deficiencies and/or financial and property obligations or b) you still have deficiencies and/or financial and property obligations.
• The process, from the time you email the PDF file, will take at least 72 hours to complete.
• Be sure to check your mail client’s Spam folder.

​Step 6

Deficiencies should be settled personally with the relevant office.
• Go to the University and enter as a visitor.
• Proceed to the Dean’s Office and ask for your partially signed clearance form.
• Go to the office where you have a deficiency or deficiencies.
• Settle the deficiency or deficiencies.
• Ask for your clearance form.
• Proceed to the remaining offices indicated in the Clearance Form to obtain the rest of the signatures; your last stop should be the University Registrar’s Office.

​Step 7

For financial and property obligations, a copy of the assessment will be attached to the email.
• Deposit/transfer the amount indicated in the assessment to any of the bank accounts of the University (if there is no need for you to go to the University to settle any deficiency).
• Email a copy of the deposit slip or proof of fund transfer to [email protected] and to your Dean’s/Principal’s Office email address using the Email Subject Line: Proof of payment – Lastname, Firstname M.I.
• Repeat step 7.

Request For Documents

Transcript of Records

  1. Write a letter request addressed to the College Dean.
  2. Get a copy of School Clearance @ Registrar’s Office (Window 6).
  3. Submit the complied school clearance together with the approved letter request, 2 pieces of 2×2 picture (white background w/ collar & sleeve) & school ID @Window 6 in order to fill up the request form.
  4. For graduates who enrolled in another school, Letter of no Objection (LNO) is required.

    UNDERGRADUATE AND INACTIVE STUDENTS WHO WISH TO REQUEST FOR ALL GRADES PRINT OUT OR T.O.R- SCHOOL CLEARANCE IS ALSO REQUIRED.

    Representatives must bring the following in claiming the documents requested:

    • Claim Slip-you are advise to read the important notes and take note of the date and time when to claim .
    • Authorization Letter– photocopy of the owner’s ID and representative’s must bring valid ID.

    CERTIFICATION, AUTHENTICATION AND VERIFICATION

    PROCESS FLOW

    1. Go to the Registrar’s Office- Window No.1. Submit the school documents (original or photocopy) that you want to be certified, authenticated, verified or “Red Ribboned” by the Commission on Higher Education (CHED). Fill-up the request, submit it to the personnel-in-charge and wait for the issuance of a charge slip. Please do not hesitate to make inquiries for appropriate guidance, whenever deemed necessary.

      3. Other degrees/courses  :TOR & Diploma only
    2. Proceed to the Accounting office. Present the charge slip and pay to the cashier the amount indicated therein(@ P400.00 per set of documents). Make sure that you obtain the official receipt obtaining the correct amount.
    3. Go back to the Registrar’s office. Submit the official receipt issued by the Accounting Office. The personnel-in-charge shall then issue a CDU Claim Stub indicating the specified date and time you are advised to come back to CDU.
    4. After completing the CDU Processes, your documents will then be indorsed by the University Registrar to CHEDRO VII for verification.
    5. CHEDRO VII will indorse and send your documents to the Department of Foreign Affairs (DFA) in Manila where the verified and authenticated documents in Red Ribbon will be claimed.

    Important Note: DFA due date is determined by CHEDRO VII.

    • Not earlier than the date and time specified in the claim stub, you are advised to come back to CDU in order to obtain the “CLAIM STUB FOR DFA AUTHENTICATION” prepared by the Registrar’s Office where the DFA due date as determined by CHEDRO VII is indicated.

    • If you choose not to go to DFA Manila for the claiming of your documents, you may instead decide to avail of a DFA authorized or accredited courier service by proceeding to any of the following:

    • DHL Ramos branch(Across Diplomat Hotel)- F. Ramos St., Cebu City

    • DHL Near Cebu Metropolitan Cathedral

    BE GUIDED ACCORDINGLY

    Diploma

    Write a request letter addressed to the College Dean for re-issuance of Diploma.

    Note: Your request letter must be approved for a re-issuance

    Payments & Discounts
    Payment Plan

    A student may choose to pay in installment up to a maximum of three (3) installments, as follows:

    • 15,000 upon enrollment..

    • Seventy percent (70%) all fees before midterm examination. 

    • Thirty percent (30%) all fees and must be fully paid before the final examination.

    Discounts

    • A student who pays in full upon enrollment, except for the Summer term, is given a five percent (5%) discount on the tuition fees. A student who may want to pay in full to avail of the discounted rate may do so up to two (2) weeks after classes start.

    • A 50% discount on tuition fees will be given to the youngest of at least 3 siblings enrolled in the semester.

    • Discounts will be given to all CDU Scholarship grants. 

    • CDU Alumni will get a 20% discount on tuition fees once they enroll again in any program the university offers excluding College of Medicine, Senior High School.
